Job summary

ComplexCare Solutions (CCS) seeks a Nurse Practitioner for a Per Diem role covering all zip codes in Lancaster County, Nebraska. You will conduct comprehensive home and telehealth assessments to support health plans nationwide, focusing on evaluating health status, gaps in care, and risk factors to improve outcomes.

Responsibilities include documenting encounters, making evidence-based treatment recommendations, coordinating with PCPs and health plans, and maintaining HIPAA-compliant records.

Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP board certification as a Nurse Practitioner.
• Maintains current CPR certification.
• Autonomous patient assessments in remote settings.
• Knowledge of CMS regulations and NCQA HEDIS guidelines.
• Experience with Medicare/Medicaid and health plan benefits.
• Excellent communication with elderly and chronically ill patients.

Responsibilities

• Review member population and document encounters.
• Conduct comprehensive home assessments and capture patient needs.
• Communicate assessment findings and recommendations to PCPs and health plans.
• Identify gaps in care and suggest management plans.
• Perform Point of Care testing and document results.
• Maintain HIPAA-compliant records and privacy standards.
• Support physician/patient relationships and coordinate care.
• Aim for at least 25 availability slots per month (≈37 hours).
