Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem home and telehealth assessments across Hall County, Nebraska. This role supports patient-centric care, documenting findings and coordinating with PCPs and health plans to improve outcomes.

The ideal candidate holds an active NP license with ANCC/AANP certification, CPS CPR, and experience with EHRs. Remote clinical work requires strong communication with elderly and chronically ill patients and adherence to CMS guidelines.

Qualifications

• Active unencumbered NP license.
• ANCC or AANP board certification in Family/Adult/Gerontology/Emergency NP.
• CPR certification current.
• Knowledge of CMS Regulations and NCQA/HEDIS guidelines.
• Ability to conduct remote assessments and document findings.
• Excellent communication with elderly and chronically ill patients.

Responsibilities

• Conduct comprehensive member assessments in home or telehealth settings.
• Document assessment findings, recommendations and follow-up plans.
• Close gaps in care (disease-specific or preventive).
• Coordinate with PCP and health plan for follow-up and referrals.
• Maintain HIPAA compliance and confidentiality.
• Perform Point of Care testing and document results.
• Maintain up-to-date knowledge of CMS/health plan policies.
