Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem, performing comprehensive home and telehealth assessments nationwide. You will evaluate health status, identify care gaps, and collaborate with physicians to develop evidence-based treatment plans. Remote practice requires autonomous decision-making, strong documentation, and HIPAA compliance.

Qualifications

• This role requires ANCC or AANP board certification as a Nurse Practitioner.
• Active, un-encumbered nursing license is required.
• Ability to practice autonomously in a remote clinical environment.

Responsibilities

• Review member health status and document encounter data in charting systems.
• Document active medical conditions per CMS/DHHS and client requirements.
• Communicate, document findings, and coordinate follow-up with PCP/health plan.
• Provide recommendations to improve chronic condition knowledge and care gaps.
• Close disease- or preventive-care gaps and escalate urgent cases per policy.
• Perform and document Point of Care testing results.
• Maintain HIPAA compliance and company procedures.
• Ensure ~37 hours per month and be available for 25 slots monthly.
