Qualifications

• Minimum of 3 years of recent clinical experience, including hospice, palliative care, or acute inpatient care.
• Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
• NP certification, licensure, and prescriptive authority for home state and any other states served.
• Basic Life Support certification required; compact RN license in home state by date of hire.

Responsibilities

• Provide remote, virtual serious illness care management across home-based, facility-based, and inpatient settings.
• Consult with medical staff to ensure optimal patient care.
• Conduct virtual assessments of patient/family/caregiver needs.
• Identify, manage, and prescribe medications and equipment as appropriate.
• Adjust care plans per Chapters policies and nurse practice acts.
• Maintain EMR access and timely documentation; coordinate with primary team for continuity of care.
• Support discharge planning with medications and discharge orders.
• Participate in QAPI activities and ongoing education for quality care.
