Chapters Health System, Inc. is seeking a Nurse Practitioner to deliver remote, patient-centered care across the continuum for hospice and palliative services in Illinois.

The role requires 3+ years of relevant clinical experience and an NP degree with national certification, plus current licensure and prescriptive authority. You will collaborate with the care team, support discharge planning, and ensure compliance with regulatory standards.

Qualifications

• Must have a Masters or Doctorate in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice.
• Basic Life Support certification and compact RN license required by date of hire.
• Shifts may vary and include nights, evenings, weekends or holidays.

Responsibilities

• Provides fully remote virtual serious illness care management across hospice, palliative care, and related programs.
• Consults with medical staff to ensure optimal patient care and efficient care delivery.
• Performs virtual assessments of patient/family needs and adjusts the plan of care accordingly.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Maintains EMR access and documents interactions in a timely manner.
• Collaborates with the primary team to ensure continuity of care and discharge planning.
• Participates in QA/QI activities to improve care practices and ensure compliance with standards.
• Keeps knowledge up-to-date to maintain clinical excellence and professional growth.
