Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for Per Diem duties to perform in-home and telehealth assessments for health plans nationwide. This remote clinical role requires an active nursing license, ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine, CPR, and CMS/HIPAA compliance.

You'll spend up to a full hour with each member to gather a comprehensive health picture, document findings, and develop evidence-based treatment plans aimed at preventing

Qualifications

• ANCC or AANP board certification as a Nurse Practitioner.
• Active unencumbered nursing license.
• Current CPR certification.
• Knowledge of CMS Regulations and HIPAA requirements.
• Ability to work autonomously in a remote setting.

Responsibilities

• Perform in-home and telehealth assessments for members.
• Document encounter findings and develop care plans.
• Close gaps in care and coordinate with PCPs and health plans.
• Ensure compliance with CMS/DHHS and client policies.
• Maintain HIPAA compliance and secure data handling.
