Job summary

SCAN is hiring an Advanced Practice Clinician to provide and coordinate primary care for members in San Bernardino County. The role includes assessing patient needs, collaborating with healthcare teams, and ensuring continuity of care.

The ideal candidate will hold a Master’s degree or equivalent, be licensed in California, and have strong skills in managing chronic diseases. This patient-facing position comes with a competitive salary, bonuses, and generous paid time off.

Qualifications

• Master’s degree in Nurse Practitioner or PA program required.
• Current California Nurse Practitioner license or PA Certification.
• Knowledge of wound care and management.

Responsibilities

• Provide care to assigned panel of members, including wellness exams.
• Collaborate with case management team for member access to services.
• Engage with healthcare professionals for chronic disease management.

Skills

Knowledge of treatment of complex chronic diseases

Strong interpersonal skills

Knowledge of HCC and ICD-10 coding

Comfortable with initiating goals of care

Adaptable and open to change

Multilingual/Multicultural

Education

Master’s degree in Nurse Practitioner Certificate

Current California Nurse Practitioner license OR California Physician Assistant Certification (PA-C)
