Qualifications

• At least 5 years clinical experience in hospital, home health, pharmaceutical, or biotech settings

• Driving is required with valid license and driving record review

Responsibilities

• Deliver remote patient education via telephonic, video, email, SMS, and secure messaging platforms
• Provide in-person support and education during territory vacancies, including home visits
• Coordinate connections to program-approved patient resources and support dialogue with healthcare providers
• Maintain documentation and uphold KPIs and compliance standards
• Collaborate with District Managers and Field Leadership; assist in onboarding and mentoring new team members
• Provide educational training to patients on disease states and prescribed medications
• Foster dialogue between patient and health care provider
• Support patients to overcome access barriers and promote behavioral change for better health outcomes
