Qualifications

Responsibilities

• Deliver remote patient education and adherence support via telephonic, video, email, SMS, and secure messaging.
• Provide in-person education during territory vacancy coverage, including home visits.
• Coordinate connections to program-approved resources and facilitate dialogue between patients and healthcare providers.
• Maintain accurate documentation, follow protocols, and uphold KPIs and compliance standards.
• Collaborate with District Managers and Field Leadership; assist in onboarding and mentoring team members.
