Job summary

Optum is seeking a Transition of Care Registered Nurse in Utah. This telephonic role coordinates care for patients moving from observation, inpatient, or post-acute settings, collaborating with patients, caregivers, and health teams to prevent readmissions and improve outcomes.

Ideal candidates will have an active Utah RN license and 3+ years in relevant nursing roles, with experience in care management and population health. Some remote work flexibility is available for Utah residents.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in Utah.
• 3+ years of clinical nursing experience, preferably in acute care, care management, case management, utilization management, discharge planning, or transitional care.
• Experience in managed care, Medicare Advantage, value-based care, care management, or population health environments.
• Demonstrates ability to assess member needs, develop care plans, and manage complex cases with minimal supervision.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Excellent written, verbal, and interpersonal communication skills.
• Ability to collaborate with providers, hospitals, CHWs, and interdisciplinary teams.
• Proficiency with EMR, case management systems, and Microsoft Office.

Responsibilities

• Assess and develop individualized care plans for discharged patients and high-risk Medicare Advantage patients.
• Conduct comprehensive assessments to identify factors impacting recovery and health outcomes.
• Outreach to eligible patients after hospitalization or post-acute care stays.
• Evaluate discharge plans and address barriers to recovery or adherence.
• Perform medication reconciliation and flag potential concerns for follow-up.
• Educate patients and caregivers to support self-management and transitions.
• Coordinate care across primary, specialty, hospital, SNF, home health, and community resources.
• Collaborate with CHWs and teams to address social determinants of health.
