Job summary

UnitedHealth Group is seeking an RN Case Manager-ED in Draper, UT to perform telephonic proactive case management for high-risk ED utilizers. The role partners with providers and community resources to address clinical and social needs and promote appropriate care settings.

Active Utah RN license required with 3+ years of clinical nursing and experience in care management or population health. Flexible remote work options within Utah and collaboration with interdisciplinary teams are key aspects

Qualifications

• Current, unrestricted RN license in Utah.
• 3+ years of clinical nursing experience in acute care, care management, case management, utilization management, discharge planning, or transitional care.
• Experience in managed care, Medicare Advantage, value-based care, care management, or population health environments.
• Ability to assess member needs, develop care plans, and manage complex cases with minimal supervision.
• Strong written, verbal, and interpersonal communication skills.
• Proficiency with electronic medical records, case management systems, and Microsoft Office applications.

Responsibilities

• Deploy RN case management support for identified high-risk and frequent ED utilizers.
• Conduct comprehensive assessments across clinical, psychosocial, behavioral health, and social determinants of health needs.
• Deliver proactive outreach, care coordination, and longitudinal member engagement.
• Reinforce PCP engagement and facilitate timely follow-up appointments.
• Provide education to members and caregivers on disease management and appropriate care settings.
• Identify and address root causes of ED utilization and care gaps.
• Connect members to Complex Case Management, Disease Management, Behavioral Health, and community resources.
• Utilize ED diversion strategies to prevent avoidable hospital admissions.
• Monitor progress and adjust interventions as needs change.
• Document assessments, interventions, care plans, and outcomes per requirements.
