Job summary

Optum is seeking a Telephonic Case Manager RN to support a diverse member population with education, advocacy and connections to resources. This remote role allows work from anywhere in the U.S. and focuses on improving health outcomes through coordinated care.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Ability to obtain multiple state and/or compact licensure within 60 days of hire.
• 3+ years of RN experience in hospital/acute care or telephonic case management for insurance.
• Demonstrated computer proficiency with data entry and Windows tools.
• Quiet workspace at home and access to secure high speed internet.

Responsibilities

• Make outbound and inbound calls to assess members' health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to assist with self-management.
• Interact with Medical Directors on challenging cases.
• Coordinate care for members.
• Educate members on disease processes.
• Encourage healthy lifestyle changes.
• Document and track findings.
• Make post-discharge calls to ensure services and resources.
