Qualifications

• Active, unrestricted Utah RN license.
• 3+ years clinical nursing experience; acute care or care management preferred.
• Experience in managed care, Medicare Advantage, or population health.
• Strong assessment, care planning and independent decision skills.
• Excellent written and verbal communication.

Responsibilities

• Deploy RN case management for high‑risk and frequent ED utilizers.
• Conduct comprehensive assessments of clinical, psychosocial and social determinants.
• Provide proactive outreach, care coordination and long‑term member engagement.
• Facilitate timely follow‑ups with PCPs and care teams.
• Educate members/caregivers on disease management and medications.
• Document assessments, plans and outcomes per regulatory requirements.
• Support utilization management and quality improvement goals.
