Job summary

Optum UT is seeking a Skilled Nursing Facility (SNF) Case Manager in Draper, UT to coordinate care for Medicare Advantage members in SNFs. You will work with facility staff, providers, and interdisciplinary teams to support safe transitions and improve outcomes.

Candidate must have an active Utah RN license and 3+ years of clinical nursing experience in care management or related fields. Remote work flexibility is offered for Utah residents.

Qualifications

• Active Utah RN license.

Responsibilities

• Manage caseload of Medicare Advantage members in SNF.
• Develop and monitor individualized care plans.
• Coordinate with SNF staff and providers.
• Participate in interdisciplinary care conferences and discharge planning.
• Monitor progress toward goals and reduce readmissions.
• Ensure timely documentation and regulatory compliance.
