Job summary

Optum UT is seeking a RN Case Manager - ED to join our team in Draper, UT. This role delivers telephonic proactive case management to high-risk ED utilizers, coordinating care across providers, caregivers, and community resources.

You will assess clinical, behavioral, and social determinants of health, develop care plans, educate patients, and support ED diversion and admission prevention while collaborating with interdisciplinary teams. Active Utah RN license required.

Qualifications

• Current, unrestricted RN license in Utah.
• 3+ years clinical nursing in acute care, care management or related fields.
• Experience in managed care, Medicare Advantage or population health.
• Ability to assess needs, develop care plans and manage complex cases.
• Strong clinical assessment, critical thinking and decision-making.
• Excellent written and verbal communication skills.
• Ability to collaborate with providers, hospitals and care teams.
• Proficiency with EMR, case management systems and MS Office.

Responsibilities

• Deploy case management for high-risk ED utilizers within assigned markets.
• Conduct comprehensive assessments across clinical, psychosocial and SDOH needs.
• Deliver proactive outreach, care coordination and longitudinal member engagement.
• Reinforce PCP engagement and ensure timely follow-up appointments.
• Educate members and caregivers on disease management and adherence.
• Identify root causes of ED utilization and care gaps.
• Connect members to programs and community resources.
• Utilize ED diversion strategies to prevent admissions.
• Document assessments, interventions, plans and outcomes.
• Support utilization management and quality improvement goals.
• Perform other duties as assigned.
