About This Role

The Nurse Case Manager - Transition Of Care is responsible for coordinating and managing care for members transitioning from various healthcare settings. This role is ideal for RNs who excel in collaborative environments and are passionate about improving patient outcomes.

• Pay: $72,800 - $130,000/year

• Schedule: Full Time, Day shift

What You’ll Do

• Assess, develop, implement, and monitor individualized care plans for discharged acute care patients and high-risk Medicare Advantage patients.


• Manage a caseload and conduct comprehensive clinical assessments to identify factors impacting recovery and health outcomes.


• Complete timely outreach to eligible patients following hospitalization or post-acute care stays.


• Evaluate discharge plans and address barriers that may interfere with recovery or treatment adherence.


• Perform medication reconciliation and identify potential medication-related concerns.


• Engage patients and caregivers in care planning, provide education, and promote self-management.


• Coordinate care across healthcare settings and providers.


• Collaborate with Community Health Workers and interdisciplinary team members to address social determinants of health.

Requirements

Education

• Current, unrestricted Registered Nurse (RN) license in Utah

Experience

• 3+ years of clinical nursing experience, preferably in acute care, care management, or transitional care


• Experience working within managed care, Medicare Advantage, or population health environments

Licenses & Certifications

• Current RN License in Utah
