UnitedHealth Group is seeking a Nurse Case Manager - Transition Of Care in Draper, Utah. You will coordinate and manage care for members moving from hospital to home or other settings, focusing on quality outcomes for high-risk Medicare Advantage patients.

The role requires an active Utah RN license and 3+ years of clinical nursing experience in acute care or care management. This is a full-time day-shift position offering a comprehensive benefits package.

Competenze

• Active Utah RN license.
• Minimum 3 years of clinical nursing experience in acute care, care management, or transitional care.
• Experience in managed care, Medicare Advantage, or population health.

Mansioni

• Assess, develop, implement, and monitor care plans for discharged acute care and high-risk Medicare Advantage patients.
• Manage caseload and perform clinical assessments to identify factors affecting recovery and outcomes.
• Conduct timely outreach to eligible patients after hospitalization or post-acute care stays.
• Evaluate discharge plans and address barriers to recovery or adherence.
• Perform medication reconciliation and identify potential medication-related concerns.
• Engage patients and caregivers in care planning, provide education, and promote self-management.
• Coordinate care across healthcare settings and providers.
• Collaborate with community health workers and the care team to address social determinants of health.
• Facilitate timely primary care and post-discharge follow-up appointments.
• Monitor patient progress during the transition period and modify interventions as needed.
