Job summary

Evergreen Nephrology is seeking a licensed practical nurse to conduct monthly CCM touchpoints, address clinical concerns, reinforce CKD care plans, and ensure accurate documentation across EHR and CCM systems. This remote role supports patients between visits and requires collaboration with a multidisciplinary team.

The ideal candidate holds an LPN/LVN license, is bilingual (Spanish/English), and has 1–2 years in ambulatory care, population health, or care coordination.

Qualifications

• Active LPN or LVN license in good standing.
• Bilingual (Spanish/English) required.
• 1–2 years’ clinical experience in ambulatory care, population health, care coordination or case management.
• Strong clinical assessment, communication and documentation skills.
• Proficient with two systems (EHR + CCM platform).

Responsibilities

• Conduct monthly CCM touchpoints with assigned patient panels.
• Review patient data before calls and reinforce care plans.
• Escalate urgent issues per triage guidelines; coordinate with providers.
• Document all interactions clearly in EHR and CCM platform.
• Coordinate follow-up, labs, and nephrology appointments to reduce missed care.
• Identify barriers to care and refer to appropriate resources.
