Monogram Health is seeking a Registered Nurse Care Manager to work with a small patient panel in home-based care, coordinating with providers and social workers to improve quality of life and slow CKD/ESRD progression.

The role emphasizes building trust, delivering patient education, managing care plans, and coordinating transitions of care with dialysis teams. Local travel is frequent; BLS is required and RN licensed in NJ; remote call duties may apply.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in the state of the posted location.
• 2+ years experience in care management or with CKD/ESRD patients.

Responsabilités

• Work closely with patients’ medical providers to develop and continually adapt care plan.
• Perform in-home care management visits to execute care management plan.
• Monitor biometric data and follow approved protocols for interventions.
• Coordinate with pharmacists and prescribers for medications.
• Perform patient health assessments and surveys as required.
• Deliver education on CKD/ESRD, dialysis and comorbidities.
• Encourage medication and treatment adherence through frequent contact with patients.
• Engage family and social support groups in education and care.
• Serve as primary point of contact during business hours.
• Provide education on medications, diet, exercise, and lifestyle choices.
• Educate patients about Advance Care Plans and ESRD treatment modalities.
• Obtain vital signs during visits and escalate concerns.
• Initiate enrolment and onboarding processes.
• Perform post-op and hospital discharge visits.
• Document updates in care management platform.
• Coordinate transitions of care with dialysis providers.
