Job summary

Evergreen Nephrology is seeking a remote Care Coordinator to support patients in the Mountain Time Zone, 8:30a–5p MST. You will engage patients, develop care plans with the NCM, and connect members to housing, transportation, food security, and community resources.

Qualifications

• 3 years of experience in health care coordination, community health work, or similar healthcare related field.
• Experience with team-based approach to care
• Excellent working knowledge of community resources
• Exceptional ability to prioritize and task-switch to meet needs of patients and overall care team
• Self-directed and motivated to resolve issues professionally and creatively
• Strong communication and teaching skills
• MS Office Suite of products including Outlook and Teams
• Ability to work effectively in a primarily remote environment with adequate home setup

Responsibilities

• Patient Engagement and Enrollment: telephonic outreach to enroll and engage patients identified for your clinical program and establish connection as primary point of contact.
• Care Coordination: develop and implement care plans addressing patient needs under the direction of the NCM.
• Social Determinants of Health: identify factors impacting members’ health and connect to resources.
• Resource Navigation: guide members in understanding and utilizing health and social services.
• Member Advocacy: identify barriers to care and address inequities.
• Collaborative Communication: maintain open lines of communication with patients and families and healthcare teams.
• Documentation and Reporting: record interactions and care plans timely and accurately.
• Education and Empowerment: provide resources to help patients navigate conditions.
