Job summary

Evergreen Nephrology in a fully remote role seeks an LPN to conduct monthly CCM check-ins, reinforce care plans, and collaborate with providers while working from home in the Pacific Time zone.

The nurse will engage patients, document accurately, review chart data, and coordinate labs/appointments. Compensation ranges from $55,000 to $65,000 annually with potential quarterly bonuses and full benefits.

Qualifications

• Active LPN or LVN license.
• 1–2 years of ambulatory care, population health, care coordination, or case management.
• Experience in remote/telehealth nursing preferred.
• Ability to work across two systems (EHR + CCM) with reliable communication.

Responsibilities

• Perform monthly CCM touchpoints and document outcomes.
• Build relationships with patients and caregivers and conduct outreach.
• Review patient data and reinforce CCM care plans.
• Escalate urgent issues per triage pathways.
• Coordinate care including labs and appointments.
• Ensure HIPAA-compliant documentation.
