Qualifications

• Minimum of 3 years of recent clinical experience, including hospice, palliative care, and/or acute inpatient care.
• Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
• National certification, licensure, and prescriptive authority in home state and any required jurisdictions.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families across the continuum of care.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments and manage medications and equipment as appropriate.
• Document all interactions in the EMR promptly and accurately.
• Collaborate with the primary team to ensure continuity of care and discharge planning.
