Job summary

Chapters Health System, Inc. is seeking a remote Night & Weekend Nurse Practitioner to deliver virtual serious illness care management to patients and families across home-based, facility-based, and inpatient settings as part of our hospice and palliative care continuum.

The role requires at least 3 years of relevant clinical experience, NP licensure and prescriptive authority in the home state, and BLS certification.

Qualifications

• Must hold a Master’s or Doctorate in Nursing from an accredited Family or Adult NP Program.
• Must hold national certification, licensure, and prescriptive authority for home state and additional states where care is provided.
• Must hold and maintain Basic Life Support and compact RN license by date of hire.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across our continuum of care.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient, family, and caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Adjusts and supports the plan of care within policy and nurse practice act.
• Assesses caregiver ability to meet patient needs and confidence in plan of care.
• Maintains access to EMR, e-prescribing systems, and required websites.
