Job summary

Chapters Health System seeks a remote Nurse Practitioner to deliver fully remote serious illness care management across hospice, palliative care, and related services. You will work with patients and families, consult with the care team, and ensure timely documentation in EMR while supporting discharge planning.

The ideal candidate has 3+ years of hospice/palliative or acute inpatient experience, an MSN/DNP in Family or Adult NP, and current licensure with prescriptive authority.

Qualifications

• Must have a minimum of 3 years of clinical experience in hospice/palliative/inpatient care.
• Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other states where care is provided.
• Must possess Basic Life Support certification and compact RN license in home state by date of hire.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across our continuum of care.
• Consults with medical staff to ensure optimal patient care in an efficient manner.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Maintains documentation in the EMR and other required systems in a timely manner.
• Collaborates with the primary team to ensure continuity of care and supports discharge planning.
