Job summary

Qualifications

• NC resident with active RN license in North Carolina.
• 2+ years of clinical RN experience required.
• Multi-state licensure welcomed and may be pursued after hire.

Responsibilities

• Assess, plan, implement and coordinate case management activities via telecommunication.
• Evaluate members' medical needs and facilitate wellness and program transitions.
• Apply clinical judgment to reduce risk factors and address barriers to care.
• Coordinate with supervisors and participate in case conferences for multidisciplinary care.
