Job summary

CVS Health is seeking a Remote RN Case Manager in North Carolina to guide members through their healthcare journey from home. This 100% remote role features Monday–Friday hours and a focus on compassionate, outcomes-driven care.

You will conduct telephonic assessments, develop care plans, coordinate with providers, and support transitions of care while upholding HIPAA and company standards. NC residency, active NC RN license, and 2+ years of nursing experience are required.

Qualifications

• Active, unrestricted RN license in North Carolina.
• Residence in North Carolina is required.
• Minimum 2 years of RN clinical experience.
• Associate Degree in Nursing is required.
• Experience in clinical settings such as ICU, ED, Med/Surg, Home Health, etc. is acceptable.
• Strong communication and member engagement skills.

Responsibilities

• Conduct telephonic assessments to identify members’ medical, behavioral, and social needs.
• Develop and coordinate personalized care plans to support overall wellness.
• Collaborate with healthcare providers and multidisciplinary teams to remove barriers to care.
• Utilize clinical judgment to address complex health conditions and improve outcomes.
• Support members through transitions of care and connect them with appropriate resources.
• Maintain compliance with regulatory and company case management standards.
