Job summary

CVS Health Corporation in North Carolina is seeking a dedicated RN Case Manager to telework from a home office. You will assess, plan, implement, and coordinate case management activities to evaluate medical needs and support overall wellness, with occasional travel to the High Point office for meetings within 30 miles.

Normal hours are 9:00am–5:30pm EST, Monday–Friday, with possible evenings and holidays. The role requires NC licensure, strong clinical experience, and the ability to navigate

Qualifications

• Active and unrestricted RN license in North Carolina.
• 2+ years of clinical RN experience.
• Willing to obtain multiple state RN licensure after hire (expenses paid).
• Preferred: CCM certification.
• Preferred: BSN.
• Experience with discharge planning and managed care is a plus.
• Proficient with Microsoft Office Suite.

Responsibilities

• Develops proactive actions to improve short/long-term outcomes and overall wellness.
• Evaluate member needs and benefit eligibility; facilitate transitions to programs.
• Apply clinical judgment to reduce risk factors and address complex health indicators.
• Assess conditions including co-morbid and multiple diagnoses affecting functionality.
• Review prior claims to assess impact on case management and eligibility.
• Assess work capacity and related restrictions; coordinate referrals as needed.
• Consult with supervisor and participate in multidisciplinary case conferences.
