Job summary

CVS Health is seeking a Remote RN Case Manager in North Carolina to guide members through their healthcare journey from home. The role offers Monday–Friday 9:00 AM–5:30 PM EST, with occasional evenings and limited travel. You will conduct telephonic assessments, develop care plans, and collaborate with providers to remove barriers to care.

The ideal candidate holds an active NC RN license, at least 2 years of RN experience, and an Associate Degree in Nursing. BSN and CCM are preferred.

Qualifications

• Active, unrestricted RN license in North Carolina.
• Must reside in North Carolina.
• Minimum 2 years of RN clinical experience.
• Associate Degree in Nursing.
• Strong communication and member engagement skills.
• Experience in any clinical setting (ICU, NICU, ED, Med/Surg, Telemetry, OR, Home Health, Hospice, LTC, Infusion, Orthopedics, etc.).

Responsibilities

• Conduct telephonic assessments to identify members’ medical, behavioral, and social needs.
• Develop and coordinate personalized care plans to support overall wellness.
• Collaborate with healthcare providers and multidisciplinary teams to remove barriers to care.
• Utilize clinical judgment to address complex health conditions and improve outcomes.
• Support members through transitions of care and connect them with resources.
• Maintain compliance with regulatory and company case management standards.
