Responsibilities

• Conduct telephonic pharmacist consultations to complete comprehensive medication reviews and adherence-focused discussions.
• Document care notes clearly and efficiently while communicating to members' primary care providers.
• Utilize claims data and MTM protocols to identify gaps in care, high-risk meds, and opportunities for therapeutic substitution.
