Job summary

Equity Staffing Group is seeking MTM Pharmacists for remote work nationwide on a 6-month contract with potential extension or conversion to FTE. You will perform telephonic comprehensive medication reviews, MTM-related adherence calls, and collaborate with members and providers to optimize therapy.

Responsibilities include documenting care notes, prioritizing HIPAA compliance, and using MTM platforms and common tools.

Qualifications

• Active pharmacist license in state of residence (unrestricted)
• 1+ year of remote work experience in a cross-functional setting
• Ability to navigate MTM platforms and multiple databases/tools
• Proficiency with Word/Outlook/Excel; strong documentation skills

Responsibilities

• Perform telephonic comprehensive medication reviews and MTM-related adherence calls
• Collaborate with members and providers to optimize therapy and disease-state management
• Document care notes clearly, concisely, and in compliance with HIPAA
• Utilize prescription data to identify gaps in care and provide recommendations
• Support program operational needs and participate in team meetings
