Qualifications

• Current and unrestricted Pharmacist license in the state of residence.
• 1+ years of remote work/independent experience in cross-functional teams.
• Ability to navigate MTM platforms and multiple databases.
• Proficient in Microsoft Word, Outlook and Excel.

Responsibilities

• Perform telephonic comprehensive medication reviews and MTM tasks.
• Document care notes and communicate with members' providers.
• Maintain HIPAA privacy and organizational policies.
• Schedule pharmacist consultations and engage members.
• Support productivity and participate in departmental meetings.
