Equity Staffing Group is staffing several MTM Pharmacists for remote work nationwide. This contract-to-potential-FTE role involves performing telephonic consultations with members to complete comprehensive medication reviews, adherence-related consults, and improve disease state management.
The Clinical Engagement Services Center team handles MTM services such as adherence, diabetes, and Retrospective Drug Utilization review.
Qualificações
- Current and unrestricted Pharmacist license in the state of residence.
- Strong knowledge of disease state management guidelines and MTM core elements.
- 1+ years of experience working remotely/in a cross-functional team.
- Ability to navigate MTM platforms and multi-task across databases.
- Proficiency with Microsoft Word, Outlook and Excel.
Responsabilidades
- Perform telephonic comprehensive medication reviews and adherence-related consultations.
- Document care and communicate to member's primary care providers.
- Schedule pharmacist consultations with program participants.
- Ensure HIPAA compliance and protect patient privacy.
- Provide patient education on medication regimens and disease states.
- Maintain accurate and concise patient care notes.
