Job summary

Equity Staffing Group is staffing MTM Pharmacists for remote work nationwide on a 6-month contract with potential extension or conversion to FTE. Start dates are 11/30/2026 or 12/07/2026.

The Clinical Engagement Services Center handles MTM, adherence, diabetes management, and related services. Pharmacists perform telephonic consultations, document care, and coordinate with members’ providers, reporting to the Operations Team Manager.

Qualifications

• Current and unrestricted Pharmacist license in the state of residence.
• Strong knowledge of MTM components and disease state management.
• 1+ years of remote work experience in a cross-functional team.

Responsibilities

• Conduct telephonic comprehensive medication reviews and MTM/adherence consultations.
• Document care provided and communicate with members' primary care providers.
• Utilize claims data to identify gaps in care and generate clinical recommendations.
• Maintain HIPAA-compliant patient information handling and documentation.
• Support team with clinical program improvements and ad hoc projects.
