Qualifications

• Current and unrestricted pharmacist license in the state of residence.
• 5 Core Elements of MTM Services.
• 1+ year of remote/independent work experience.
• Proficiency with MTM platforms and MS Office.

Responsibilities

• Engage members telephonically and schedule pharmacist consultations.
• Prioritize privacy and HIPAA compliance.
• Use claims data to generate MTM recommendations.
• Assess regimens for unnecessary or unsafe medications.
• Educate patients on medication regimens and disease states.
• Document patient care notes clearly and concisely.
• Collaborate with clinical teams and participate in meetings.
• Support special projects as needed.
