Community Care of North Carolina Inc is recruiting a Member Engagement Specialist for a primarily remote role based in Cary, NC. Qualifications

• High School diploma or GED required.
• Experience in telephonic call centers or health care settings preferred.
• Knowledge of medical terminology is highly preferred.

Responsibilities

• Conduct continual telephonic outreach to identified and referred members focused on engaging members with care management services.
• Receive and manage referrals from data reports and clinical staff for screening and linkage to care management.
• Complete and document member screenings and refer clinical needs to appropriate care management staff.
• Schedule members for face-to-face or telephonic encounters for follow up.
• Meet daily and monthly program productivity and engagement metrics.
• Notify supervisor of any issues with referrals or care management assignments.
• Adhere to HIPAA regulations and department guidelines.
• Attend departmental and corporate meetings.
