Job summary

Community Care of North Carolina Inc is seeking a Health Concierge to support the Care Management team in achieving key performance indicators, including closing care gaps and reducing hospitalizations. This non-clinician role may work remotely within regions or on-site at CCPN practices.

You will handle referrals, eligibility checks, screenings, member education, care coordination, and navigation across multiple EHRs.

Qualifications

• High school diploma or GED required.
• 2–4 years of experience in a health care setting required.
• Knowledge of patient or clinical data systems.
• Excellent verbal and written communication skills.
• Proficiency with MS Office and internet tools.

Responsibilities

• Receive and document referrals into the Care Management platform.
• Outreach, engagement, and scheduling of members for Care Managers.
• Verify eligibility and demographic information.
• Assist with care coordination and system navigation.
