Qualifications

• High School diploma or equivalent.
• 1+ years in healthcare, community outreach, or similar.
• 1+ years in call center or front-office healthcare role.
• Strong organizational and time management skills.
• Comfortable with performance metrics and outreach targets.
• Empathy and cultural competence with diverse populations.
• Proficient with basic computer systems, including EHRs.

Responsibilities

• Handle inbound and outbound calls with professional courtesy and within SLA.
• Schedule and reschedule appointments, send reminders, and confirm visits.
• Manage shared inboxes, queues, and worklists for timely responses.
• Document information accurately in the EHR/EMR and other systems.
• Support members with scheduling, transportation, pharmacy, and care navigation questions.
• Escalate issues per protocols to protect member safety and service quality.
• Maintain HIPAA compliance and confidentiality.
