Job summary

Pro Care Innovations, LLC is seeking a motivated Care Team Coordinator (CTC) to join our Chronic Care Management (CCM) team. This role emphasizes building patient relationships, coordinating care, and ensuring high-quality documentation, with a remote work-from-home option.

The CTC will manage an assigned CCM panel, conduct monthly outreach, and collaborate with providers to support continuity of care while complying with HIPAA and regulatory standards.

Qualifications

• Medical Assistant certification or equivalent healthcare experience required.
• Strong communication and relationship-building skills.
• Excellent organizational skills and attention to detail.
• Ability to work independently while managing a patient caseload.
• Comfortable with technology and electronic documentation systems.
• Experience in CCM, care coordination, case management, primary care, or population health is highly desirable.
• Passion for helping patients improve their health outcomes through ongoing engagement and support.

Responsibilities

• Build and maintain ongoing relationships with an assigned panel of CCM patients.
• Conduct monthly patient outreach and care coordination activities.
• Document patient interactions accurately and timely in accordance with CCM requirements.
• Monitor patient needs, care plan updates, and follow-up items.
• Address patient questions and concerns with professionalism and compassion.
• Coordinate with providers and internal team members to support continuity of care.
• Ensure all documentation meets regulatory and billing compliance standards.
• Support patient retention and engagement efforts within the CCM program.
• Maintain confidentiality and comply with HIPAA requirements.
