Job summary

TimeDoc Health, Inc. is hiring Care Coordinators to support patients with chronic conditions via Chronic Care Management (CCM). This is a phone-based, productivity-driven role in a remote environment with a fixed schedule.

You will conduct calls, educate patients, coordinate with providers, and document interactions to meet CCM service goals. Strong communication and EMR proficiency are required.

Qualifications

• 1–3 years of CMA/RMA or LPN experience.
• Active certification or multistate license for LPNs.
• Strong verbal and written communication in English.
• Comfortable being on the phone most of the day.
• Experience with EMR/EHR systems.

Responsibilities

• Conduct monthly CCM phone calls with assigned practice.
• Make 30–50 outbound calls per day.
• Complete required patient interactions to meet CCM goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with providers, pharmacies, and community resources.
• Identify gaps in care and social determinants of health.
• Create and update personalized care plans in the EMR.
• Accurately document all patient interactions.
• Maintain HIPAA compliance and confidentiality.
