Job summary

TimeDoc Health, Inc. is seeking a Care Coordinator to support chronic care management (CCM) for patients in a fully remote role. You will conduct monthly CCM calls, educate patients on condition management, and coordinate care with providers, pharmacies, and community resources.

The position offers a fixed schedule, 40 hours per week, Monday–Saturday, with a remote work setup. The ideal candidate has CMA/RMA or LPN experience, active certification or licensure, strong English communication, and

Qualifications

• 1–3 years of CMA/RMA or LPN experience.
• Active CMA/RMA certification or LPN license.
• Strong verbal and written English communication.
• Experience with EMR/EHR systems.
• Organized, reliable, and able to work independently in a remote setting.

Responsibilities

• Conduct monthly CCM phone calls with the assigned practice.
• Make 30–50 outbound calls per day.
• Complete required patient interactions to meet CCM service goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with provider offices, pharmacies, and community resources.
• Identify gaps in care and social determinants of health.
• Create and update personalized care plans in the EMR.
• Accurately document all patient interactions.
• Maintain HIPAA compliance and patient confidentiality.
