Job summary

Arcadia Care Bloomington is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid case-mix documentation and care plans, optimizing PDPM and RUG categorization. You will oversee the MDS/PPS documentation cycle and coordinate with nursing, dietary, social services, restorative and physicians to support reimbursement.

The role requires RN or LPN licensure, long-term care experience, restorative certification within six months, and strong attention to detail.

Qualifications

• Active RN or LPN license in state.
• Experience in Long Term Care required.
• Restorative certification within six months of hire if not already certified.
• Proficient in English (reading/writing).

Responsibilities

• Assess health status and care level for new admissions.
• Ensure timely completion of all MDS assessments (PPS, quarterly, annual).
• Maintain schedule of MDS assessments with correct reference dates.
• Complete care plans on admission, quarterly, as needed.
• Schedule and conduct resident care conferences as required.
• Oversee restorative nursing measures on care plans.
