Job summary

Arcadia Care is seeking an MDS Coordinator to manage accurate Medicare/Medicaid MDS and care plan documentation for optimal PDPM/RUG reimbursement. The role integrates nursing, dietary, social recreation, restorative and physician services to assure complete assessments.

The ideal candidate is an RN or LPN with LTC experience and restorative certification soon after hire; strong English communication skills are required.

Qualifications

• RN or LPN with current state licensure.
• Long-term care experience required.
• Restorative certification within six months of hire.
• Ability to read, write, speak and understand English.

Responsibilities

• MDS assessment and documentation including PPS Medicare, quarterly, annual and significant changes.
• Complete care plans on admission, quarterly and as needed per regulatory timeframes.
• Schedule and conduct resident care conferences; ensure MDS reviews are completed before conferences.
• Oversee restorative nursing measures and ensure they are reflected on the care plan.
