Job summary

Arcadia Care in Illinois is seeking an MDS Coordinator to ensure accurate and timely Medicare/Medicaid case-mix and care plan documentation to assure appropriate reimbursement. You will monitor MDS and PPS/PDSN processes to optimize PDPM and RUG categorization.

You will oversee scheduling and completion of MDS assessments, care plans on admission and quarterly, and coordinate with nursing, dietary, social recreation, rehabilitation and physicians to ensure comprehensive assessments and

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care Experience required.
• Restorative Certification required within six (6) months of hire if not already certified at time of employment.
• Ability to read, write, speak and understand the English language.

Responsibilities

• MDS assessment and documentation for PPS/PDPM reimbursement.
• Assesses health status and care level of new admissions.
• Schedules and maintains MDS assessments with correct reference dates.
• Completes care plans on admission, quarterly and as needed, per regulatory timeframes.
• Coordinate care planning with nursing, dietary, social recreation, restorative and physician services.
• Oversee restorative nursing plan alignment on care plans.
