Job summary

Arcadia Care is seeking a qualified MDS Coordinator to ensure accurate Medicare/Medicaid case‑mix and care plan documentation for optimal reimbursement. You will lead MDS reviews, coordinate PPS submissions, and integrate nursing, dietary, social and restorative services.

RN or LPN with active license required; LTC experience and restorative certification within 6 months preferred. Hybrid schedule with some remote work offered.

Qualifications

• Registered Nurse or Licensed Practical Nurse with a current unencumbered state license.
• Long Term Care experience required.
• Restorative certification within 6 months if not already certified at time of employment.
• Ability to read, write, speak and understand English.

Responsibilities

• MDS assessment and documentation responsibilities for new admissions and ongoing reviews.
• Care planning and resident assessment according to regulatory time frames.
• Attend and coordinate interdisciplinary care conferences.
• Oversee restorative nursing program alignment with care plans.
