Job summary

Arc at Sangamon valley is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid case-mix documentation and care plans. You will integrate nursing, dietary, social and physician services to optimize PDPM/RUG categorization and reimbursement.

The role requires LTac experience, an active RN or LPN license, and restoration certification within six months. Excellent English communication is essential for cross-disciplinary coordination.

Qualifications

• Must hold an active RN or LPN license in good standing.
• Long Term Care experience is required.
• Restorative Certification must be obtained within six months of hire.
• Proficient reading, writing, speaking and understanding of English.

Responsibilities

• Perform MDS assessments and ensure timely documentation.
• Develop and update care plans per regulatory timeframes.
• Coordinate care conferences and interdisciplinary reviews.
• Oversee restorative nursing program documentation on care plans.

Education

RN or LPN license (active)

Restorative Certification (within 6 months)
