Job summary

Arcadia Care is seeking an experienced MDS Coordinator in Illinois to oversee MDS assessment, PPS, and care plan documentation for accurate reimbursement. You will integrate nursing, dietary, social recreation, restorative and physician services to ensure complete and compliant records.

Responsibilities include conducting MDS reviews, coordinating care conferences, and ensuring timely documentation; a Long Term Care background and restoratives certification within six months are preferred.

Qualifications

• Active RN or LPN license required.
• Long Term Care experience required.
• Restorative Certification within six months of hire.

Responsibilities

• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Maintain an accurate schedule of all MDS assessments with reference dates during resident stay.
• Complete care plans on admission, quarterly and as needed per regulatory time frames.
• Schedule and conduct resident care conferences and coordinate with interdisciplinary team.
• Oversee restorative nursing measures reflected on care plans.

Education

Registered Nurse license (RN) or Licensed Practical Nurse license (LPN)

English proficiency

Restorative Certification (within 6 months)
