Job summary

Arcadia Care Aledo is seeking a dedicated MDS Coordinator to manage MDS/PDPM documentation and care plan processes for residents, ensuring accurate reimbursement and regulatory compliance. The role integrates nursing, dietary, social, restorative, rehabilitation, and physician services.

Ideal candidates hold an active RN or LPN license, with long-term care experience and restorative certification within six months.

Qualifications

• RN or LPN licensure required.
• Long Term Care experience required.
• Restorative Certification required within six months of hire if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• MDS assessment and documentation to ensure accurate PPS/MDS submissions and regulatory compliance.
• Care plans completed on admission, quarterly, and as needed with thorough documentation.
• Coordinate care conferences and interdisciplinary reviews with regulatory adherence.
• Oversight of restorative nursing measures reflected in care plans and restorative program coordination.
