Job summary

Arcadia Care is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid MDS and care plan documentation, optimizing PDPM and RUG categorization, and overseeing PPS submissions. The role integrates nursing, dietary, social recreation, restorative, rehabilitation and physician services to assure proper assessment and reimbursement.

You will conduct ongoing MDS reviews, develop care plans on admission and quarterly, and coordinate interdisciplinary conferences.

Qualifications

• Licensed RN or LPN in good standing.
• Minimum long-term care experience.
• Restorative certification required within 6 months of hire if not already certified.
• Ability to read, write, speak and understand English clearly.

Responsibilities

• MDS assessments and documentation for Medicare/Medicaid.
• Create and update care plans on admission, quarterly, and as needed.
• Coordinate resident care conferences and interdisciplinary reviews.
• Oversee restorative nursing measures on care plans for reimbursement.
