Job summary

Talentify is seeking a skilled MDS Coordinator to ensure timely Medicare/Medicaid case-mix and care plan documentation for reimbursement. You will oversee MDS/PPS submissions and integrate interdisciplinary services to optimize PDPM and RUG categorization.

Responsibilities include scheduling MDS assessments, completing care plans, conducting care conferences, and coordinating restorative nursing across nursing, dietary, social, and medical staff to ensure compliant, accurate records.

Qualifications

• RN or LPN licensure in good standing
• Long Term Care experience required
• Restorative Certification within six months of hire if not already certified
• Ability to read, write, speak and understand English

Responsibilities

• MDS assessment and documentation for residents
• Care planning on admission, quarterly and as needed
• Schedule and conduct resident care conferences; ensure MDS reviews are completed
• Oversee restorative nursing program and ensure documentation is reflected in care plans
