Job summary

You will conduct face-to-face and remote assessments, ensure documentation accuracy, and participate in conferences with supervisors.

Qualifications

• Bachelor's degree in human behavioral sciences or related field.
• 5+ years of LTSS experience or combination of education and experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 75% within assigned territory.

Responsibilities

• Manage intake, assessments, and development of support plans; ongoing case management and documentation.
• Maintain confidentiality per policy and handle sensitive information.
• Conduct quarterly and six-monthly contacts with members; update assessments and care plans.
